My Medical Clinic is pleased to announce that Covid vaccinations are now available through us.

We will need to schedule 2 -3 weeks in advance for the shots. We will be likely be doing the Pfizer vaccine although that is not guaranteed.

For Employees

Step 1

 
  • Covid vaccine appointments are available on Wednesdays, Thursdays, and Fridays. Please choose an available slot below. The clinic is open for regular business hours all days of the week. For non Covid vaccine appointments, please click here.
  • Please choose an available appointment on one of these days and follow the steps to complete your request for a Covid vaccine appointment.
  • After you have successfully submitted the request, you should receive an email confirming your request.

Step 2

 
  • Within 24 to 48 hours, you should receive an email confirming the exact date and time of your scheduled Covid vaccine appointment.
  • You will need to show up at My Medical Clinic loctated at 1560 Beam Avenue, Suite F, Maplewood, MN 55109 (unless told otherwise) at the date and time of your scheduled Covid vaccine appointment.

  • Please show up at My Medical Clinic located at 1560 Beam Avenue, Suite F, Maplewood, MN 55109 (unless told otherwise) at the date and time of your scheduled Covid vaccine appointment with
    • the completely filled out COVID-19 Vaccine Screening and Agreement form. This is a mandatory requirement.
    • a form of identification like driver's license or state ID card, if available. 

Pfizer is a 2 shot vaccine series that provides 95% protection against covid infection and near 100% for hospitalizations and death. There is a 3 -4 week gap between the 1st and 2nd doses. There is a small risk of allergic reactions after the vaccine including anaphylaxis. We will call 911 if there is an emergency arising from the vaccine administration for patient safety at the cost of the employee/ employer. The clinic will not bear the cost of the emergency transportation or medical complications arising due to vaccination. This is an Emergency Use Authorization by FDA to control the coronavirus pandemic. Every employee is advised to review the fact sheet on vaccines available at the Minnesota department of health MDH website. By agreeing to the vaccination you agree to the terms of vaccination including the risks and benefits and are choosing by free will to get vaccinated.

For Employers

You need JavaScript enabled to view it. or calling 651-340-1445 with basic details of the name of company, number of employees that have confirmed interest and intent on receiving the vaccine. We can provide shots to family members too of the employee if planned in advance (16 and older only). The company needs to send details of each employee to include

if any These details need to be for each employee or family member of the employee who wants the vaccine. My Medical Clinic intends to provide the vaccination on-site if the employee number is >20 but alternatively we can set up a time at the clinic for your employees to receive a shot. All vaccination records will be entered into Electronic Medical Record and then uploaded to the state of MN vaccination registry. Significant amounts of pre-work, as well as post-work, will need to be done by the clinic as the state requires us to be using up all the allocated vaccines within a few days. Employer to send information by Tuesday of every week for vaccination plan for the following next week Wednesday and Thursday.

Week 1: Employer needs to send details by Tuesday for vaccination planning for Wednesday of Week 2. The clinic has to place allocation requests by Wednesday afternoon. Any request from the employer after that will be planned for Week 3.

Cost to the employer: Vaccinations are available for free to all employees. Employers will have a setup/ administration fees. Please contact with details details on number of employees, business address and industry, contact details.
