Coronavirus (COVID-19) Screening Questions

Please answer the following questions to screen for COVID-19:

1. Have you traveled to any of these locations in the last 14 days?

  • Domestically in the United States
  • China
  • Iran
  • South Korea
  • Italy
  • Japan

2. Have you had contact with anyone with confirmed COVID-19 in the last 14 days?

3. Have you had any of these symptoms in the last 14 days?

  • Fever greater than 100
  • Difficulty breathing
  • Cough

4. Are you currently experiencing fever over 100, difficulty breathing or cough?

If you answered yes to question 1 and-or 2, please call your primary care provider or your State Department of Health for further direction.

Please do NOT visit a medical facility unless you are severely ill. Please call ahead and let any medical facility know why you are coming. Meanwhile, do not get close to anyone with a compromised immune system or other underlying condition.

If you answered no to questions 1 and 2, but yes to numbers 3 or 4, please contact your healthcare provider if symptoms are severe.
